PRESCHOOL/PRE-KINDERGARTEN Here is a link to a .pdf file for the preschool/pre-Kindergarten physical forms. These forms are required for all children in the preschool/pre-Kindergaten classes and are provided by the state. This form can be downloaded and taken to your doctor's office at your chid's 3-year or 4-year old physical. Preschool/pre-Kindergarten students are required to have current (within the last 12 months) physicals on file in the school office.
Preschool and Pre-kindergarten children must be up to date with their immunizations. Please click this link to get a copy of the immunization form. If you want to exempt your child from immunization, click here for a copy of that form.
KINDERGARTEN Kindergartners entering school in the fall of 2011 must have a Certificate of Immunization with the required up to date immunizations. Kindergartners do not have to have a physical before entering school, but it is strongly encouraged.

A family may be exempt from immunizations based on medical or religious reasons. In this case, the exemption form must be filed with the school (exemption form).

If your student is planning to go out for a sport during the next school year, please use this sports physical form when you get your student's physical.
